INTRODUCTION

If your employer elected this service, you would have received information during open enrollment or at your hire date. If you have questions, contact your H.R. Department or TAI.

The mbi Flex Convenience® debit card ("Card") makes spending your pre-tax "Flex-Dollars" easy as 1, 2, 3 because it works just like a credit card. Based on your plan benefits, you may use the card to pay for prescriptions, glasses, doctor visits, child care, parking and mass transit expenses.

The Card eliminates the two time payment problem with flexible spending accounts where your contribution is deducted from your salary and you have to pay for services before you are reimbursed. That's because your expenses are paid immediately at the time of each transaction by an employer-funded account. Best of all, there are no claims to file! However, we urge you to keep receipts in the event we need to verify eligible expenses.

As the contract administrator, each day, TAI electronically reviews all Card activity. The nature of your expense remains confidential.

 

USING THE CARD - The Card is Mailed to Your Home Address

1. Sign the back of the Card before use;
2. Use the Card for the services indicated below; and
3. Save all receipts.

Health Care Clinics,
Durable Medical Supply Stores,
Dependent Care Facilities
Parking Lots
Transit Agencies

IMPROPER CARD USAGE

The Card may not be used at ATMs or used to purchase liquor, beer, food, gasoline, non-prescribed medicine, catalog orders, etc. The Card will be denied if used at an ineligible location. Because the Card is to be used solely for expenses authorized by Internal Revenue Code sections 125, 129 and 132, it is understood that ineligible expenses will be reimbursed to your employer. Further, if a pattern of misuse is detected, your Card will be canceled. Either TAI or your employer will notify you concerning any misuse of the Card.

In the event merchants or providers do not accept MasterCard®, you must submit a claim form with receipts to TAI for reimbursement.

FUNDING YOUR ACCOUNT

For commuter and dependent care expenses, your accounts are funded by the first day of each month so use the Card accordingly. As an example, if you elected $75 per month for commuter expenses, you may not charge $80 on the Card nor may you prepay for next month's transit expenses. If you enrolled in the Dependent Care Reimbursement Account ("DCRA"), you may not charge more than your monthly contribution to pay for services.

On the other hand, if you enrolled in the Health Care Reimbursement Account ("HCRA"), your account is funded up to the amount you elected for the year effective from the first day of any new plan year (the same arrangement without the Card).
